Services Provided

Prior to your loved one moving into Crest View Adult Family Home they need to have an up to date assessment done by a qualified licensed nurse. Elaine Nerland, RN has been our trusted Nurse Delegator for 16 years and can help with this if needed. That assessment and visit with the resident and family will help us get a detailed history to make the transition into Crest View as smooth as possible. Our team will write a detailed care plan for all caregivers to follow which will be reviewed by Tanya and the DPOA to make sure it is the best possible plan of care for your loved one. We offer the following services, plus additional services based on the residents needs.

Personal Care and Hygiene Assistance

We can help with all personal care needs from minimal assistance to total assistance. Here are some of the ways we can help your loved one.
Bathing no less than 3 times per week unless otherwise requested.

  • Toileting, including incontinent care,
  • Catheter and colostomy care
  • Dressing
  • Assisting with putting on TED hose/ therapeutic braces/ prosthetics
  • Shaving
  • Oral care
  • All bed linens changed weekly or as needed
  • 1 person transfers/positioning

Mobility, Exercise, and Activities

If your family member has limited mobility, our home was designed with that in mind. We have assistive devices throughout our home. Along with wide walkways and doors making it wheelchair (excluding electric) accessible to all with an easy floor plan.

  • Grab bars in all bathrooms
  • Step in showers
  • Roll in shower chair/benches
  • Transfer poles if required
  • Slide boards/ sit to stand assist
  • High rises/commodes

We often work with Home HealthCare agencies who come into our home if a resident is in need of PT or OT. With their guidance we work alongside them to help with building strength and independence throughout the week.
We also have exercise activities based on the resident’s wants and needs.

Medication Assistance

Under the supervision of our nurse delegator we are able to assist at all levels with medication. Including blood sugar checks and insulin for diabetics, as prescribed by their doctor. Our resident manager works closely with our residents healthcare providers and pharmacy to make certain they take all of their medication as prescribed. Our team is constantly monitoring for adverse effects from medication or any changes and reporting to their healthcare team to resolve the issue. Our team is educated on the medication we give and is able to tell each resident what medication they are taking and why if they have any questions. Our team will also order all prescribed medications when they are needed from the pharmacy so the family will not have to worry about this and can just enjoy their time with their loved one when they come to visit.

Feeding Assistance and Nutritious Meals

We offer nutritious home cooked meals and pay special attention to dietary restrictions and needs. Common dietary needs we accommodate for are gluten-free, low-sugar, NAS, allergies, and vegan diets. We also are able to alter the texture of foods and consistency of fluids to meet their dietary needs. We offer feeding assistance as well.

Levels of Care We Offer

Our experienced Certified Nursing Assistants are able to care for all levels of care needs. We care for people who are in need of minimal assistance, such as you would find in an Assisted Living Facility, in a home-like environment. All of our staff have specialty training in Dementia and Mental Health Training, giving us extra knowledge and tools to work with Alzheimer’s Disease and Dementia residents, as well as end of life care. You can find comfort in knowing you will have to relocate your loved one again as long as we can safely meet their needs.

Home Health Therapies

We work closely with our resident’s physicians. At times, due to weakness, change in condition, or illness, some residents need and benefit from external Therapy Services. We have several Home Therapy Companies that offer care to our home. If your loved one needs PT, OT, or Speech Therapy they will be able to receive it in the comfort of their home. Our staff works with these Therapists to continue their therapy throughout the week.

Hospice also comes to our home if it is time for their services for comfort and care. This makes things more comfortable as your loved one will no longer have to seek medical care outside of their home.

Hairdressing and Nail Care

Our staff take pride in helping our residents look and feel their best. After they have their hair washed we will dry and style to their liking. We also have a professional hairdresser that comes to our home every couple of months, or as needed, for hair cuts.
Staff also makes sure nails are kept clean and trimmed. If your loved one likes to have their nails polished we are happy to help them with that. Diabetics may require the assistance of a Podiatrist.

Outings

Residents are free to leave Crest View with their family or friends for outings if they wish. Occasionally Crest View staff have outings to local events if they would like to go. This can include trips to the local farmers market, the fair, Christmas lights and many of our local restaurants.

What We Don’t Do

By law, we are not allowed to provide the following:

  • Intravenous therapy
  • Sterile dressing change
  • Physically restrain or lock residents in
  • Give medications, prescribed or over the counter, that are not part of the treatment plan from the residents doctor unless in a written order
  • We are not allowed to have more than 6 residents in each home
