Before your loved one joins Crest View Adult Family Home, a thorough assessment by a qualified licensed nurse is essential. Elaine Nerland, RN, our trusted Nurse Delegator for 16 years, can assist with this process if needed. This assessment, along with discussions involving the resident and family, will provide us with a comprehensive history, ensuring a seamless transition into Crest View. Our dedicated team will craft a detailed care plan tailored to your loved one’s needs, meticulously reviewed by Tanya and the Durable Power of Attorney (DPOA) to ensure it is optimal. We provide a range of services, with additional offerings customized to meet each resident’s specific requirements.

Personal Care and Hygiene Assistance

We provide comprehensive assistance for all personal care needs, ranging from minimal support to complete assistance. Here are the specific ways we can support your loved one:

  • Bathing, scheduled at least 3 times per week unless otherwise requested.
  • Toileting, including care for incontinence.
  • Management of catheters and colostomies.
  • Dressing assistance.
  • Help with putting on TED hose, therapeutic braces, and prosthetics.
  • Shaving assistance.
  • Oral hygiene care.
  • Weekly changing of all bed linens, or more frequently as required.
  • Assistance with transfers and positioning by one caregiver.

Our goal is to ensure your loved one receives the highest standard of care in every aspect of daily living.

Mobility, Exercise, and Activities

Our home is designed to accommodate individuals with limited mobility, featuring a range of assistive devices and accessible features. Here’s what we offer:

  • Grab bars in all bathrooms
  • Step-in showers
  • Roll-in shower chairs/benches
  • Transfer poles as needed
  • Slide boards and sit-to-stand assists
  • High rises and commodes

Our wide walkways and doors ensure easy wheelchair access (excluding electric chairs) throughout our thoughtfully planned layout.

In collaboration with Home HealthCare agencies, we support residents needing physical therapy (PT) or occupational therapy (OT). Working closely with these professionals, we help residents build strength and regain independence throughout the week.

Additionally, we provide tailored exercise activities based on residents’ preferences:

  • Engaging puzzles
  • Interactive games
  • Creative crafts
  • Enjoyable outings
  • Baking and cooking sessions
  • Celebrations for birthdays and holidays

Medication Assistance

Under the expert oversight of our nurse delegator, we offer comprehensive medication assistance tailored to individual needs. This includes administering medications at all levels, conducting blood sugar checks, and administering insulin for diabetic residents as prescribed by their doctor. Our resident manager collaborates closely with healthcare providers and pharmacies to ensure all medications are taken as prescribed.

Continuous monitoring by our team ensures prompt detection of any adverse medication effects or changes, promptly reported to healthcare providers for resolution. Our dedicated team is well-versed in the medications administered, providing residents with clear explanations of their prescriptions upon request.

Additionally, we handle all medication orders directly with the pharmacy, alleviating concerns for families so they can focus on quality time with their loved ones during visits.

Feeding Assistance and Nutritious Meals

Levels of Care We Offer

Our team of experienced Certified Nursing Assistants (CNAs) excels in providing comprehensive care tailored to individuals requiring varying levels of assistance. Whether your loved one requires minimal support akin to an Assisted Living Facility or specialized care in a comforting home-like environment, we are here to support them.

All our staff undergo specialty training in Dementia and Mental Health, equipping them with the expertise and tools necessary to care for residents with Alzheimer’s Disease, Dementia, and those needing end-of-life support. This specialized training ensures that we can meet the unique challenges and needs of these residents with compassion and skill.

You can rest assured that our commitment to quality care means your loved one can stay with us comfortably without the need for relocation, as long as we can safely meet their evolving needs.

Home Health Therapies

We maintain close collaboration with our residents’ physicians to ensure comprehensive care. In cases where residents experience weakness, changes in condition, or illnesses, they may benefit from external Therapy Services. Our facility partners with several Home Therapy Companies, allowing residents to receive Physical Therapy (PT), Occupational Therapy (OT), or Speech Therapy in the comfort of their own home. Our staff coordinates closely with these therapists to ensure continuity of care throughout the week.

Additionally, we facilitate hospice services within our home when needed, providing comfort and specialized care. This approach ensures that your loved one can receive necessary medical care and support without the need to leave the familiar surroundings of our facility.

Hairdressing and Nail Care

Our staff take pride in enhancing the well-being of our residents, ensuring they look and feel their best. We offer hair washing, drying, and styling according to their preferences. Additionally, a professional hairdresser visits our home periodically for haircuts.

Our attentive staff also maintain clean and trimmed nails for residents. For those who enjoy having their nails polished, we gladly provide assistance. Diabetic residents may require care from a Podiatrist for foot health needs.

Outings

Residents at Crest View are welcome to accompany their family or friends for outings as they desire. Additionally, our staff organizes occasional outings to local events such as the farmers market, the fair, Christmas lights displays, and various local restaurants.

What We Don’t Do

By law, we are not allowed to provide the following:

  • Intravenous therapy
  • Sterile dressing change
  • Physically restrain or lock residents in
  • Give medications, prescribed or over the counter, that are not part of the treatment plan from the residents doctor unless in a written order
  • We are not allowed to have more than 6 residents in each home
